Supervision Agreement
The following agreement is between SJP Wellbeing and you, the person booking the appointment.
By confirming your booking, you agree to follow the
directives outlined in this supervision agreement and to maintain professional guidelines outlined by our respective ethical principles and codes of conduct, laws, and regulations.
1. Purpose, Goals, and Objectives of Professional Supervision:
Monitor and promote the welfare of clients seen by the Supervisee;
Promote the development of the Supervisee's professional identity and competence;
Support and nurture the growth of the supervisee.
2. Context and Content of Professional Supervision
Group professional supervision via Zoom.
3. Method of Evaluation
Feedback will be provided each session. Records will be limited to session details and major issues relevant to the professional supervision of the case. Professional supervision notes (if kept) may be shared with the Supervisee at the Professional supervisor's discretion and upon request of the Supervisee.
Basic reporting will be provided to the employer, pertaining to the types of issues presented in supervision, the number of participants, and any necessary risk assessment handover. No confidential client or supervisee information will be shared unless there is a risk to client or supervisee.
4. Duties and Responsibilities of Professional Supervisor-Supervisee
A. Professional supervisor will:
Encourage ongoing professional education.
Challenge the Supervisee to validate the approach and techniques used.
Monitor basic micro-skills and advanced skills, including transference and countertransferences.
Provide alternative approaches for the Supervisee.
Intervene where client welfare is at risk.
Ensure ethical guidelines and professional standards are maintained.
Provide consultation when necessary.
Discuss and provide professional development such as psychoeducation.
B. Supervisee:
Uphold ethical guidelines and professional standards.
Discuss client cases with the aid of written case notes and video/audiotapes.
Validate diagnoses made and approaches and techniques used.
Be open to change and alternative methods of practice.
Consult their Professional supervisor or designated contact person in cases of emergency.
Implement Professional supervisor directives in subsequent sessions.
Maintain a commitment to supervise education and their profession.
Maintaining confidentiality of all client material and discussing information that could identify the client.
Cooperating with ethical problem-solving procedures.
The supervisee remains responsible for their own actions at all times.
Functioning professionally and fairly in a group setting.
Consulting your Supervisor immediately if in doubt about any aspect of service delivery, or where clients are at risk.
Practicing legally at all times.
Maintaining any professional memberships required to perform your role.
When in a group supervision setting, the supervisee must maintain that all information discussed within the group falls under privacy and confidentiality. Client or information about other supervisees are not to be released or discussed outside of the group.
5. Procedural Considerations
a. Supervisee's written notes, diagnoses, action plans, or videos may be reviewed in sessions.
b. Issues related to the Supervisee's professional development will be discussed.
c. It is understood that important and seminal issues experienced in the professional setting will be raised and addressed in professional supervision. Failure to raise such issues in a reasonable timeframe will be considered a breach of contract.
This contract is subject to revision at any time, upon request by either Professional Supervisor or Supervisee.
Cancellation policy
As practicing supervisees, there is an expectation by your professional supervisor to commit fully to the professional supervision process, and this includes keeping appointments. Unexpected emergencies can be discussed on an individual basis.
Please be aware that a maximum of 6 spots are available in each session if you do not cancel your booked session with enough time, you may be limiting someone else's ability to attend.
Contact outside of appointment times:
There may be occasions when you need to consult your professional supervisor outside of appointment times. Your workplace team leader should be your first connection. Within reason, text, email, or phone calls will be responded to if you have questions or need to debrief 1:1 about things that came up in supervision.
Agreement:
We agree, to the best of our ability, to uphold the guidelines specified in the professional supervision contract and to manage the professional supervisory relationship process in accordance with the above.
Indemnity:
You are not an employee or an agent of SJP Wellbeing and as such you will indemnify any person working within SJP Wellbeing against any action arising from any client in relation to your activities.
